CERTIFICATE OF LIFE A certificate of life is a certificate that confirm that and individual was alive at the time of its creations. It is generally used for collect pensions and/or salaries. To request the certificate of existence in life, it is necessary to fill in the form at the following link ... and ask for an appointment at the Offices of the Representation in Italy of the Kurdistan Regional Government. To request the certificate of existence in life, the following documents are required: • Kurdish identity card (national card) Iraqi citizenship, passport; • copy of the pension card; • copy of an Italian identification document (or of the country in which you reside); 2 passport-size photos NB: to request the Certificate of Life, it is necessary the presence at the Offices of the Representation in Italy of the Kurdistan Regional Government.
